Logistic回归模型分析显示,感染性ALI(OR=2.332,95%CI:1.372~3.964)、APACHE II评分(OR=3.105,95%CI:1.662~5.802)、Alb(OR=0.532,95%CI:0.347~0.813)、PT(OR=2.487,95%CI:1.327~4.661)、西维来司他钠治疗(OR=0.429,95%CI:0.231~0.795)是ALI患者近期转归不良的影响因素(P均<0.05)。

Abstract Objective: To analyze the influencing factors of poor short-term outcomes in patients with acute lung injury (ALI) based on propensity score matching (PSM). Methods: The clinical data of 116 patients with ALI were retrospectively analyzed. According to their outcomes at 28th day after admission, the patients were divided into a poor outcome group (41 cases) and a good outcome group (75 cases). The general data of the two groups before matching were compared. PSM was used to match the data of the two groups, and the general data of the poor outcome group (34 cases) and the good outcome group (34 cases) after matching were compared. Based on the matched data, a Logistic regression model was used to analyze the influencing factors of poor short-term outcomes in ALI patients. Results: The poor outcome rate of the 116 ALI patients was 35.34%. Before matching, the age, proportion of diabetes, and lung ultrasound score in the poor outcome group were higher than those in the good outcome group, while the oxygenation index was lower than that in the good outcome group (all P< 0.05). Before and after matching, the proportion of infectious ALI, C-reactive protein (CRP), and acute physiological status and chronic health evaluation II (APACHE II) score in the poor outcome group were higher than those in the good outcome group, while albumin (ALB) and the proportion of sivelestat sodium treatment were lower than those in the good outcome group, and prothrombin time (PT) and activated partial thromboplastin time (APTT) were longer than those in the good outcome group (all P< 0.05). Logistic regression analysis showed that infectious ALI (OR=2.332, 95% CI: 1.372-3.964), APACHE II score (OR=3.105, 95% CI: 1.662-5.802), ALB (OR=0.532, 95% CI: 0.347-0.813), PT (OR=2.487, 95% CI: 1.327-4.661), and sivelestat sodium treatment (OR=0.429, 95% CI: 0.231-0.795) were influencing factors for poor short-term outcomes in ALI patients (all P<0.05). Conclusion: Based on PSM analysis, infectious ALI, APACHE II score, ALB, PT, and sivelestat sodium treatment are influencing factors for poor short-term outcomes in ALI patients.
