  摘要 目的:探究血清微小RNA(miR)-30c-5p及T-钙黏蛋白13(CDH13)水平与急性缺血性脑卒中(AIS)患者病情程度和炎症反应的相关性。方法 :选取132例AIS患者为研究对象(AIS组),根据入院时美国国立卫生研究院卒中量表(NIHSS)评分分为轻度组53例、中度组44例和重度组35例,另纳入同期体检且一般资料与AIS组患者相匹配的志愿者132例为对照组。采用实时荧光定量逆转录聚合酶链反应(qRT-PCR)测定所有受试者血清miR-30c-5p、CDH13水平;酶联免疫吸附法测定超敏C-反应蛋白(hs-CRP)、肿瘤坏死因子(TNF)-α、白细胞介素(IL)-1β水平;比较AIS组与对照组血清miR-30c-5p、CDH13及hs-CRP、TNF-α、IL-1β水平;比较不同病情程度AIS患者上述5项指标;Spearman相关性分析血清miR-30c-5p、CDH13水平与NIHSS评分之间的相关性;Pearson相关性分析血清miR-30c-5p、CDH13水平与hs-CRP、TNF-α、IL-1β之间的相关性。结果:与对照组比较,AIS组患者血清miR-30c-5p水平显著降低(P<0.05),CDH13及hs-CRP、TNF-α、IL-1β水平显著升高(P均<0.05);与轻度组比较,中度组与重度组患者血清miR-30c-5p水平显著降低(P<0.05),CDH13及hs-CRP、TNF-α、IL-1β水平显著升高(P均<0.05),且重度组miR-30c-5p水平显著低于中度组(P<0.05),CDH13及hs-CRP、TNF-α、IL-1β水平显著高于中度组(P均<0.05);Spearman相关性分析显示,miR-30c-5p与NIHSS评分呈负相关(r=-0.511,P<0.05),CDH13与NIHSS评分呈正相关(r=0.698,P<0.05);Pearson相关性分析显示,血清miR-30c-5p与hs-CRP、TNF-α、IL-1β呈负相关,血清CDH13与hs-CRP、TNF-α、IL-1β呈正相关(P均<0.05)。结论:AIS患者血清miR-30c-5p、CDH13水平与病情程度及炎症反应均密切相关,测定血清miR-30c-5p、CDH13水平可评估AIS患者病情。

    Abstract Objective: To explore the correlation between serum microRNA (miR)-30c-5p and T-cadherin 13 (CDH13) levels and the severity of disease and inflammatory response in patients with acute ischemic stroke (AIS). Method: A total of 132 patients with AIS were selected as the research subjects (AIS group). According to the National Institutes of Health Stroke Scale (NIHSS) score at admission, the patients were divided into mild AIS group (53 cases), moderate AIS group (44 cases) and severe AIS group (35 cases). Additionally, 132 volunteers who underwent physical examinations during the same period and whose general data were matched with those of the AIS groups were included as the control group. The levels of miR-30c-5p and CDH13 in the serum of all subjects were determined by real-time fluorescence quantitative reverse transcription polymerase chain reaction (qRT-PCR). The levels of high-sensitivity C-reactive protein (hs-CRP), tumor necrosis factor α (TNFα), and interleukin (IL)-1β were determined by enzyme-linked immunosorbent assay. The levels of serum miR-30c-5p, CDH13, hs-CRP, TNF-α, and IL-1β in the AIS groups and the control group were compared. The above 5 indicators in AIS patients were compared among different disease severity levels. Spearman correlation analysis was conducted to examine the correlation between the levels of serum miR-30c-5p and CDH13 and the NIHSS score. Pearson correlation analysis was performed to investigate the correlations between the levels of serum miR-30c-5p and CDH13 and hs-CRP, TNF-α, and IL-1β. Results: Compared with the control group, the levels of serum miR-30c-5p and CDH13 in the AIS groups were significantly decreased (P< 0.05), and the levels of CDH13, hs-CRP, TNF-α, and IL-1β were significantly increased (all P< 0.05). Compared with the mild AIS group, the serum miR-30c-5p levels in the moderate AIS and severe AIS groups were significantly decreased (P< 0.05), while the levels of CDH13 and hs-CRP, TNF-α, and IL-1β were significantly increased (all P< 0.05). Moreover, the miR-30c-5p level in the severe AIS group was significantly lower than that in the moderate AIS group (P< 0.05), and the levels of CDH13, hs-CRP, TNF-α, and IL-1β were significantly higher than those in the moderate AIS group (all P< 0.05). Spearman correlation analysis revealed that miR-30c-5p was negatively correlated with the NIHSS score (r=-0.51, P< 0.05), while CDH13 was positively correlated with the NIHSS score (r=0.698, P< 0.05). Pearson correlation analysis showed that serum miR-30c-5p was negatively correlated with hs-CRP, TNF-α, and IL-1β, while serum CDH13 was positively correlated with hs-CRP, TNF-α, and IL-1β (P< 0.05). Conclusion: The levels of miR-30c-5p and CDH13 in the serum of AIS patients are closely related to the severity of the disease and the inflammatory response. Measuring the levels of miR-30c-5p and CDH13 in the serum can be used to assess the condition of AIS patients.
