| 摘要 二甲双胍作为经典降糖药物，由于其降糖效果确切、安全性良好，兼具成本效益优势、良好的药物可及性以及明确的长期心血管获益证据，被国内外权威指南一致推荐为2型糖尿病（T2DM）的一线首选药物和联合治疗的基础用药。我国约80%的T2DM患者于非内分泌科室就诊，跨学科规范化用药至关重要。本文回顾了二甲双胍的临床应用历程和最新国内外指南推荐，介绍二甲双胍在不同T2DM人群及合并特殊疾病状态（如妊娠期高血糖、多囊卵巢综合征）的规范用药，旨在为非内分泌科室医师的临床实践提供参考，助力实现“以患者为中心”的糖尿病全程管理。 |

| Abstract As a classic antidiabetic drug, metformin has been consistently recommended as the first-line preferred agent and foundational medication for combination therapy in type 2 diabetes mellitus (T2DM) by authoritative domestic and international guidelines, owing to its well-established glucose-lowering efficacy, favorable safety profile, cost-effectiveness, excellent drug accessibility, and proven long-term cardiovascular benefits. Approximately 80% of T2DM patients in China seek medical care in non-endocrinology departments, making interdisciplinary standardized prescribing critically important. This article reviews the clinical application history of metformin and the latest recommendations from domestic and international guidelines, and introduces the standardized use of metformin in different T2DM populations and those with special disease conditions (such as hyperglycemia in pregnancy and polycystic ovary syndrome), aiming to provide clinical practice references for physicians in non-endocrinology departments and facilitate the achievement of patient-centered comprehensive diabetes management. |
